Fees & Refund Policy 

1. Why do you charge a fee to submit a Request Form? We charge a fee for processing forms and applications to cover administrative costs and time-intensive labor that isn’t reimbursed through insurance or public funding.

2. Do you change a one-time fee. Yes, each Request Form submission requires a fee.

3. Do you allow refunds. Ask Medicaid Florida has a No Refund policy.

Request Form

1. What is my case number? Each form submission generates a case number. Case numbers include your last name, the first 2 letters of your first name followed by the date. Example: Terry Smith 05/02/2025 = case number Smithte0502202512.

 

2. What if I submit multiple forms? Each submission has its own unique case number.  

3. How do you respond to Request Form submissions? We respond to all form submissions via email. A Team member may call you for more complexed resolutions. 

4. How long does it take to resolve my case? Most cases are resolved within 3-5 days depending the complexity.

Personal Data

1. How long will you store my Request Form data? Up to 24 months from the submission date. Please read the "Personal Information We Collect" section here for additional information. 

Sales Receipts

1. Will I receive a sales receipt? Yes, you will receive a receipt for every Request Form you submit to the email you provide on the Form. Look for an email from Ask Medicaid Florida with a Subject Line titled "Thanks for Shopping with us". Your receipt will include an Order #, Date, Payment Details, Total Cost and Contact info. 

2. How to find my charge on my Credit or Debit Card. Charges on your Credit or Debit Card will appear as "Ask Medicaid Florida" or "Ask Medicaid".
