Florida Managed Medical Assistance (MMA) health plans

Florida Medicaid offers several Managed Medical Assistance (MMA) health plans through the Statewide Medicaid Managed Care (SMMC) program. These plans provide medical coverage to eligible Florida Medicaid recipients.

As part of Florida's SMMC 3.0 program, which began February 1, 2025, the state reorganized its managed care regions and contracted with new health plans.

The Agency for Health Care Administration is responsible for administering the Statewide Medicaid Managed Care (SMMC) program.  Most Florida Medicaid recipients are enrolled in the SMMC Program. The SMMC program has three components, the Long-Term Care (LTC) program, the Managed Medical Assistance (MMA) program, and the Dental Program.

On February 1, 2025, the Agency for Health Care Administration (Agency) implemented the new Statewide Medicaid Managed Care (SMMC) 3.0 program. The Agency entered into new contracts with health and dental plans that will greatly benefit enrollees and providers.

The new SMMC 3.0 program is focused on several core enhancements:

  • Family Focused Plans and Programs

  • Significant New Ways to Improve Health

  • Fully Integrated Incentives for Quality Performance

  • New SMMC Regions and Plan Structure
